The script uses jQuery to create a small form at the end of the body of the page. By clicking the button you are sending the page's info here. The results will appear in a new window and will look exactly like this page.

All you have to do is paste it below the jQuery script declaration. If you page does not use jQuery, you can install it by pasting the following line right above the script (replace the "//" in the link's start with http:// if you are trying it on an html file and not a live page.

Google usually gets your head title and uses it at the title of the search result. Make sure that it is clear and to the point. If it is bigger than ~55 characters, Google will truncate it with ellipsis. Use the preview above to check that your title looks good and that its meaning is not cut in half.
The page url appears right below the title. This makes it even more important to have clean and readable links. You should avoid non-standard characters. These get encoded and make your link unreadable. It is also possible that Google uses breadcrumbs from your page here.
The page description appears below the link in Google results. This is a nice opportunity to help users decide quickly about your site . Google checks your meta description to see if it is relevant to the rest of your page. If not, they might use a more relevant part of your page.
Again, if it is bigger than ~155 characters, Google will truncate it with ellipsis. Use the tool above to make sure your point is not cut in half.
These days meta keywords are an underdog. Google admitted not using them any more. Bing said that may use them negatively. Some people advocate not using them at all. Our opinion is to use max 5 very relevant keywords.
